Report Rabies Vaccinations

Rabies vaccination reporting is required by ordinance and is an important way to track compliance with rabies vaccination laws. Many citizens also appreciate the convenience of having their documentation submitted for them.

PetData works with veterinarians to make reporting as easy as possible. The information that is required is the same as what is on a standard rabies vaccination certificate. Reports may be submitted three ways:

• By email
• By mail
• By fax

Reports are due by the 10th of the month following that in which the vaccinations were given.

Report License Sales

Please contact PetData for reporting supplies.

1. Fill out one reporting form for each month of license sales.
2. Retain the 2nd copy of the reporting form for your records.
3. Submit the top copy of the reporting form plus copies of each license and full payment by the 15th of the month following the license sale.
